01. Consultation and health teaching
We send a registered nurse to visit the patient at home to obtain a comprehensive physical, and social history on the patient. They also assess and treat any emergent issues. Then in collaboration with the healthcare team, a plan of care is formulated and put into action.
02. Complex continuing care
Our team of highly trained professionals are able to assist patients in managing an assortment of medical conditions that require ongoing assessments, treatment, and evaluation.
03. Chronic health management
Our team of registered and practical nurses are able to assist with chronic health issues i.e. wound dressing changes, insulin checks, medication administration, and assessment.
04. Homemaker or caregiver relief
We help support patients that need assistance in activities of daily living i.e. light cleaning, organization, meal prep, toileting, and medication reminders.
